Extended Case Management Program

**currently suspended due to 2011 budget shortfalls

The Extended case management program provides one year of extended support to families after they exit into permanent housing.  This program includes:

Weekly contact with Haven Case Manager

Continued counseling and guidance

Assistance with budget planning and maintenance

Assistance with long term goal setting

Continued participation in the Daily Living Skills program
